Media Release Agreement

Should you choose to engage with any of my services, these are the terms and conditions:
- Permission to Use Media
I, [Client’s Full Name], hereby grant Susanne Cesana, Empowered Lifestyle LLC, and her representatives the irrevocable and unrestricted right to use my name, image, likeness, voice, written testimonial, and/or video recording in connection with her business and marketing materials. This permission applies to, but is not limited to:

- Testimonials (written, audio, or video)
- Photographs or video recordings
- Coaching session excerpts (if applicable)
- Live event recordings or participation in webinars
- Intended Use of Media
I understand that my media may be used for the following purposes:

- Website, social media, and online marketing
- Coaching program materials (including courses and client resources)
- Promotional materials such as advertisements, emails, and brochures
- Public presentations, webinars, or educational content
I acknowledge that my participation is voluntary, and I will not receive compensation for the use of my media.
- Waiver & Release of Claims
I release Susanne Cesana, her business, affiliates, and employees from any and all claims, liabilities, or damages arising from the use of my media. I waive the right to inspect or approve the final version of any materials in which my likeness or testimonial appears.
I understand that this release applies indefinitely unless I revoke it in writing. However, I acknowledge that revocation will not affect materials already in use or published prior to my request.
- Rescheduling Policy
Susanne Cesana will not disclose sensitive personal health details beyond what I voluntarily share in my testimonial or media. If I wish to remain anonymous or use only my first name, I will indicate this below:
